Who should make a report?
There are two (2) different types of reports that you can make:
- Giving praise for an employee that has performed his/her duties above and beyond what is expected
- Voicing a Complaint/Concern about an incident that has happened in the facility, whether or not you were a witness.
Any person that has information regarding any incident in the facility is mandated to report it so an investigation can be done. If you are an employee you should also report any violations to company policies, safety, fraud, sexual harassment, theft, abuse etc.
Although you can make this report anonymous, we encourage you to provide us with some personal information in the event that the investigator will need more details in order to assure a fair and complete investigation.
